Waiver of Liability: By my participation in this event, I freely acknowledge, understand and accept the following terms and conditions. I hereby state I have conditioned myself to participate in the event I have chosen. I, for myself, my executors, administrators and assignees, do hereby release and discharge the LAP for a LAP officials, sponsors, and volunteers from damages or injuries suffered by my participation in said event. I also authorize LAP or LAP officials to utilize photographs and videotape of my participation in the event for any and all purposes. By signing my name on the application below, I hereby certify that I have read all the terms and conditions of this release and do intend to be legally bound thereby.
